Transition
-
Beginning at age 16 or earlier if appropriate, the IEP team must consider and develop a transition plan within the student’s Individualized Education Program (IEP). There are many things parents and students should think about, plan for, and know when preparing for this very important IEP meeting. Please visit DESE's website for information and resources related to transition planning.
